This Privacy Policy (hereinafter the “Policy”) outlines how Centre Médical Longévité Inc. collects, uses, discloses, retains, and destroys your personal information. To facilitate reading, we will use the terms “we” or “the clinic” from time to time in the Policy.

We will not be able to provide you with healthcare services or hire you.

Who will have access to your personal information?

Physicians, healthcare professionals provided by CIUSSS: nursing assistants, nurses, social workers, nutritionists, pharmacists, and secretaries, all strictly in the performance of their respective duties.

For our employees, clinic management and personnel responsible for human resources and accounting, strictly in the performance of their duties.

How will we share your personal information with third parties?

We share your personal information with DME DSQ, Bonjour Santé for appointment confirmation notifications, Petal MD, and our billing agency for RAMQ.

For our employees, according to the means provided by government rules.

How long will we retain your personal information?

For the duration of the physician-patient relationship, in accordance with the rules established by the Law and the College of Physicians of Quebec.

For our employees, for the duration of the employment relationship and in accordance with the laws and regulations applicable to human resource management within a company.

What are the risks associated with the processing of your personal information?Incidents of privacy breaches may occur (see the definition above).
Where will we store your personal information?

We store your personal information in the electronic medical record system of our supplier, which is approved by the Ministry of Health and Social Services. Billing to the Régie de l’assurance maladie du Québec is done through the billing agency FMP. For our employees, in our electronic personnel management systems, those of secure and encrypted online payroll service providers.

5. SECURITY MEASURES AND INHERENT RISKS
We follow security standards in our industry to protect the information we collect and receive. Specifically, we have appropriate physical, technical, computer, and administrative protection measures in place to protect your personal information from a privacy breach.

However, despite these measures and considering the inherent risks associated with the use of computer systems, we cannot guarantee the security and confidentiality of the information you transmit to us, and you do so at your own risk. If you have reason to believe that personal information has been compromised, please contact us using the contact information provided above.

6. HOW AND UNDER WHAT CONDITIONS WILL WE SHARE YOUR PERSONAL INFORMATION FOR PUBLIC SAFETY OR CRIMINAL PROSECUTION?

6.1 In cases of serious risk of death or serious injury
We may disclose personal information we hold to protect an individual or group of individuals when there is a reasonable belief that a serious risk of death or serious injury, including disappearance or an act of violence, including a suicide attempt, threatens that individual or group, and the nature of the threat requires urgent action.

The information may then be communicated to the individuals exposed to this risk, their representative, or anyone who may provide assistance. Only the information necessary for the purposes of the communication will be disclosed.

6.2 Law Enforcement
We may also disclose information to the Director of Criminal and Penal Prosecutions or to a person or group responsible for preventing, detecting, or prosecuting crime or violations of laws when the information is necessary for prosecution of an offense under a law applicable in Quebec.

Finally, we may disclose information to a police force when it is necessary for planning or executing an intervention tailored to the characteristics of a person or situation, in either of the following cases: 1° the police force intervenes, at our request, to provide assistance or support in the provision of services to an individual; 2° we act in cooperation or partnership with the police force in the context of mixed psychosocial and police intervention practices.

7. ARE YOUR PERSONAL INFORMATION TRANSFERRED OUTSIDE OF QUEBEC?
We may transfer your personal information to service providers we engage in our operations. Therefore, your personal information may be transferred outside of Quebec, for example, to the cloud to ensure data redundancy, i.e., to ensure their integrity as indicated in Article 1 of the Policy.

8. WHAT ARE YOUR RIGHTS REGARDING YOUR PERSONAL INFORMATION?
The law grants you various rights regarding your personal information. You have the following rights:

  • Access: the right to inquire whether we hold personal information about you and, if so, to request access to that personal information.
  • Rectification: the right to request correction of any incomplete or inaccurate personal information we hold.
  • Withdrawal of consent: the right to withdraw your consent to the communication or use of the personal information held.
  • Restriction or refusal of access: the right to request that a specific individual or category of individuals specified by you not have access to one or more pieces of information you have identified.
  • Complaint: the right to file a complaint with our privacy officer as identified in Article 1 in connection with this Policy or to file a complaint with the Commission d’accès à l’information du Québec if you believe a violation of privacy laws has occurred. We do not use any technology that makes fully automated decisions based on personal information.

13. DESTRUCTION OF PERSONAL INFORMATION
Personal information is destroyed in accordance with the regulations issued by the College of Physicians of Quebec, applicable laws in Quebec regarding human resource management, and applicable laws in Quebec regarding personal information protection.

We may, as needed, modify the Policy to reflect changes in our business processes or in the law.
